Driving Qualifications For SFG |
- Drivers must have at least two years of commercial
driving experience with a Class A License in the
vehicle being asked to be insured.
- Driver must have two years of own liability
experience or four years of commercial driving
experience.
- If owner/operator, cannot have more than two
moving violations within the last three years.
- DUI's and careless/reckless convictions have to be
more than seven years old to consider.
- Drivers cannot be younger
than 23 or older than 65, but can consider a driver up
to age 70 with a long form medical approved by the
insurance company.
- If driver is between 23 and 25 must have a clean
driving record.
- Driver cannot have more than one moving violation
within the current driving year.
- Driver cannot have more than
one chargeable accident within the last three years
and cannot have more than one chargeable accident and
one moving violation within the past three years but
both not in the current year.

All quotes
We now need a complete breakdown of all commodities
hauled. 100% dry freight is no longer acceptable. Only
15% general Dry Freight.
If a policy has lapsed please include the reason why on
the cover page of the fax so we have this information
prior to being asked for it by Chartis.
New Ventures (Accounts with less than two years primary
liability coverage, Question # 1)
New Ventures
One power unit
- owner must be the only driver.
During the first year the account is limited to an
increase of power units to double the original #: one
can go to two and two can go to four. If the account
grows more than double we will consider that the
risk/exposure has significantly changed and will send a
cancellation notice adhering to the appropriate state
requirements.
Require detailed 5 year work history in the explanation
box and ask if the owner has owned a trucking operation
in the last 10 years.
Endorsements
(adding a power unit)
When there is a net gain in the
# of power units that are being added by endorsement an
MVR is required for each additional power unit and must
be reviewed for acceptability. Several large losses have
involved drivers where a MVR was not reviewed due to the
additional power unit issue, the power unit was
appropriately added by endorsement but the details of
driver assigned to that vehicle were absent. If the MVR
is not included the vehicle can not be added.
Non-owned Equipment
We will not offer physical
damage coverage for any equipment that is not owned by
the named insured. This new requirement does not apply
to Lien Holders but to third party individuals or
companies that own a piece of equipment that is operated
by the named insured.
Florida Accounts
Our loss experience in Florida
has been highly unprofitable. Effective immediately, all
accounts garaged in South Florida that are intrastate
will be declined. If they are interstate operations,
mileage reports will be required to determine the
percentage of Florida miles. If more than 50% of the
miles are in Florida then the account will be declined.
If mileage reports are not available, the account will
be declined. |

each vehicle MUST be provided within 30 days of
binding.
- Copy of down payment check and premium

Form 194 needs to be completed when requiring either
a
state or federal filing. All numbers must be shown for the necessary
filing to be done.
- Loss Runs currently valued
for past 4 years if 5 or more vehicles
- Financial Statements or Federal
Tax Returns for the past 2 years (11 or more vehicles).

The Lexington Truck Broker
Liability policy is a hybrid primary liability policy
that addresses the truck broker's general liability
arising out of the ownership and use of a truck by the
motor carrier of a brokered cargo move, in conformity
with a written truck brokerage agreement.

Payment Options to
help avoid cancellations and loss of account: |
Autopay - The ClassicPlan Autopay system sets the insured up to
have the monthly payment automatically deducted from their
checking account. Complete the Autopay (see
Auto Payment Form) form, attach a void check
from the account the payments are to be deducted, and submit to
ClassicPlan. This option is available anytime during the term of
the loan.
Pay by Phone - The insured or producer can call the ClassicPlan
customer service department with their checking account
information and submit a payment.
Pay by Fax - The insured or producer can fax a copy of the check
made payable to ClassicPlan to submit payment. The fax number is
909-628-5490.
Pay by Web - The insured or producer can visit our web site at
www.classicplan.com and submit a payment using their checking
account.
Western Union Quick Collect - The insured can visit any Western
Union office to submit a payment using the Western Union Quick
Collect system. They will need the funds to cover the payment
and their ClassicPlan loan number.
Cancellation Delay - The producer can call or fax ClassicPlan
and request that the mailing of the cancellation notice to the
carrier(s) be delayed. In this instance ClassicPlan can delay
the mailing to the notice up to 4 days (depending on weekends,
holidays, etc.) after the cancellation date.
Payment Guarantee - The producer can call or fax ClassicPlan and
guarantee payment on the account(s). ClassicPlan will stop the
cancellation process and the producer must submit their trust
check for the payment(s).
