If a Driver falls into the categories below as acceptable, no call is necessary to underwriting, just forward the driver information to us.

• No major violations in the past five (5) years

• No more than two (2) minor violations in the past three (3) years with zero (0) at-fault accidents or no more than one (1) at-fault accident with one (1) minor violation

• Two (2) at-fault accidents in three (3) years will not be acceptable

• All drivers operating light or medium vehicles exclusively, must be age 21 or over

• All drivers operating heavy or extra-heavy trucks or tractor-trailer units must be age 23 or older with at least two (2) years of like-vehicles experience and Class A CDL

• All drivers operating passenger transport vehicles that require a CDL license must have at least 2-years of like experience.  (CDL for 2 years)

• Drivers over the age of 70 will be reviewed on a case-basis and need underwriting approval

Minor Violations:

• Failure to yield

• Speeding

• Illegal passing

• Defective equipment where points are assessed

• Stop sign/stop light violation

• Careless operation

• Improper turn

• Following too closely

• Any moving violation where points are assessed

 Major Violations:

• Drag racing or speeding contest

• Speeding in excess of 20 miles per hour over the speed limit

• Driving under the influence of alcohol or narcotics (DWI or DUI)

• Open alcohol containers

• Reckless, careless, improper/negligent driving

• Refusal to take alcohol test

• Illegal possession of alcohol or narcotics in a motor vehicle

• Fleeing or attempting to elude a police officer

• Revocation or Suspension of License

Annual Billing

• Any policy is eligible for this billing option.

Semi- Annual (Installment fee $8)

• Any policy with more than a $500 annual premium is eligible.  We require 50% down at inception.  The final installment is due 150 days after the inception date.

Quarterly (4-Pay, Installment fee $8)

• Any policy with more than $1000 in annual premium is eligible for this billing option.  25% due at inception and the remaining installments will be due 60, 150, and 240 days after the inception date.

Monthly (10-Pay, Installment fee $8)

• Any policy with more than a $3000 annual premium is eligible for this option.  25% is due at inception and the balance is spread over nine (9) installments beginning 30 days after the inception date and every 30 days thereafter.

Basic Requirements:

• Up to 2 Power Units and 2 Trailers

• No drivers under age 23 or over 70

• No Hazardous Material Hauling

• New Ventures are acceptable as long as they have at least 4 years of CDL experience
