  • 1-4 Units TiQuote (Simplified application)

  • 1-25 Units Small Fleet Application (2 page Chartis application, 4 page worksheet and Acord 194)
     

  • (1-4 Units or 1-25 Units) Current MVR within 30 days.

Driving Qualifications For Chartis
  1. Drivers must have at least two years of commercial driving experience with a Class A License in the vehicle being asked to insure.
  2. Driver must have two years of own liability experience or four years of commercial driving experience.
  3. If multiple vehicles on schedule each driver cannot have more than three moving violations within the last three years.
  4. If owner/operator, cannot have more than two moving violations within the last three years.
  5. DUI's and careless/reckless convictions have to be more than seven years old to consider.
  6. Drivers cannot be younger than 23 or older than 65, but can consider a driver up to age 70 with a long form medical.
  7. If driver is between 23 and 25 must have a clean driving record.
  8. Driver cannot have more than one moving violation within the current driving year.
  9. Driver cannot have more than one chargeable accident within the last three years and cannot have more than one chargeable accident and one moving violation within the past three years.

These documents MUST be provided in order to bind. Without any one of them, Chartis will NOT bind the risk.

If you are binding 1-4 units, items 1-8 are required. For more than 4 units all items are required.

  1. 1-4 Units TiQuote (Simplified App)
    1-25 Units Small Fleet (Chartis complete application) sign and date at least 1 day prior to date of binding (cannot bind on same date)
    If risk is an Intrastate risk, will need state of authority filing number, the MC and DOT #s.  If a new risk, the application for filing.
    If an Interstate risk, need the schedule B or the quarterly IFTA reports.
  2. State of domicile UM form and (PIP if applicable) signed and dated by your client. Call for the correct UM#. Click here for your states forms.
  3. Terrorist Rejection Form signed and dated (if rejecting terrorist coverage). If applicant chooses to take the coverage, nothing is required.
  4. (1-4 Units) Sample Experience & Loss Letter when no loss runs are available for non-fleet.
    Also, if no loss runs are available, a signed letter stating the liability loss and driving experience for the past five years is acceptable.
    (5 or more units) must have currently valued loss runs for the past four years.
  5. Current MVRS valued within the last 30 days
    All drivers must have at least two years of verifiable CDL driving experience. A signed letter will be acceptable for one to four units with name of prior employer.
  6. Copy of Premium Finance Agreement and Down Payment Check unless you are using Quick Release
  7. Copy of Vehicle Registration Form or apportionment card for each vehicle MUST be provided within 30 days of binding.
  8. Copy of down payment check and premium finance agreement unless quick release has been arranged with your premium finance company.
  9. Acord Form 194 needs to be completed when requiring either a state or federal filing. All numbers must be shown for the necessary filing to be done.
  10. Loss Runs currently valued for past 4 years if 5 or more vehicles
  11. Financial Statements or Federal Tax Returns for the past 2 years (11 or more vehicles).
