¨     Current MVR, within 30 days

¨     Current annual financials for the past two years

¨     Current list of equipment, where garaged and what value at each location

¨  Show if they have a safety director, a risk manager, and a loss control person. If they do give us their resumes, along with a copy of their safety and loss control programs

WORKERS' COMPENSATION (CHARTIS)

n     Chartis that can provide quotes for WC. Chartis's minimum premium in California is $20,000. The minimum is lower in other states ($10,000). The quotations are provided on a net commission basis to you.

n      The Chartis WC program is a statutory program and is available in California. The following is needed:

n      Accord Applications 125, and 130 completely filled out

n      Three years of currently valued loss runs

n      Current WCRIB worksheet

n      Current annual financials

n      Supplemental application (see downloadable)

n     The TPA approach allows for the larger risks to bear some of the risk through either a deductible or SIR which ever serves the appetite of your client

OCC/ACC AND CONTINGENT LIABILITY

n       The occupational/accident program is for the owner/operator who wants coverage while driving under contract as an independent driver. This coverage is for occupational and accidental injuries while on the job.
